I will have one-on-one conversations so I can listen to what matters most to you and ask the questions you should have answers to
I will create your custom Peace in Preparation Plan
Joined by your loved ones, you will have access to a guided and comprehensive family meeting so that you may share and discuss your decisions in a safe and supportive way. This can be done in person or virtually.
After the family conference, you will be given a carefully crafted checklist to work through at your leisure.
Finally, I will check to make sure your questions are answered and the checklist is complete.
Gain Peace of Mind
Unfortunately, dying well is not a given. As a palliative care physician, my work is to help support people in health crises. I talk about their future so that they can experience more control in an uncontrollable situation. Most often, however, I witness how late these conversations occur and the stress they place on families to make difficult decisions quickly when deep conversations have never happened.
Even when advance directives have been completed, the job is not done if it is not followed by a real dialogue surrounding the available choices. Believe me when I say that when you are at your sickest is not when you want to start..
Frequently Asked Questions
Is this different from the advance directive I completed with my lawyer? Yes. The advance directive/living will written decades ago generally falls short in a healthcare crisis. This is an opportunity to have a deeper, value-based discussion that is attuned to your wishes, alldone while your family is present.
Will I have to share my medical records? No, I am not your treating physician and therefore will not be diagnosing or prescribing for your specific medical conditions.
Isn’t this something that my primary care doctor or specialist does? Unfortunately, not all physicians are trained (or have time) to do this work during a busy office visit. This consultation allows unfettered time to discuss what is important to you.
About Dr Guerry
Roshni Guerry, M.D. is Associate Professor of Clinical Medicine at University of Pennsylvania where she is the HVC, Lung and Renal Clinic Outpatient Palliative Care Director at the Perelman Center for Advanced Medicine.
Dr. Guerry holds a bachelor's degree in chemistry from Williams College and completed her medical degree at University of Rochester School of Medicine in 2006. She continued her training at University of Rochester to complete a combined Internal Medicine and Pediatric Residency followed by a fellowship in Hospice and Palliative Medicine.
Dr Guerry was selected to the Delaware Business Times “40 Under 40” in 2016, named the Delaware Quality of Life Committee’s “Outstanding Physician in Palliative Care” for 2018, and honored as Delaware’s Top Doctor in 2020. Most recently, Dr Guerry was the University of Pennsylvania Department of Medicine Outstanding Service Award recipient in 2023.
